In keeping with the Illinois Governor’s order 20-2021, the Piatt County Nursing Home has implemented a mandatory vaccination policy effective 9/10/2021 requiring COVID- 19 vaccination(s) for all employees. In accordance with Piatt County Nursing Home’s duty to provide and maintain a workplace that is free of known hazards, we are adopting this policy to safeguard the health of our employees and their families, our customers and visitors, and the community at large from infectious diseases that may be reduced by vaccinations. In making this decision, the executive leadership team reviewed requirements from Federal Mandates and CMS guidelines.
All employees must receive the 1st round of vaccination by 9/19/2021. Any employee who fails the vaccination must submit for weekly testing. Piatt County Nursing Home may require individuals to seek and pay for their own testing based on the availability of testing materials within the facility. Individuals seeking an exemption from this requirement for medical or religious reasons should complete a request for accommodation form and submit the form to the human resources department.
Piatt County Nursing Home will pay for all vaccinations and the time spent receiving the vaccinations.
